A DOT Substance Abuse Professional (SAP) is a qualified clinician who evaluates employees who have violated DOT drug and alcohol regulations. The SAP determines education and treatment recommendations and provides follow-up evaluation to confirm the individual has successfully complied with the SAP process.
You need a DOT-SAP evaluation after a DOT drug or alcohol violation, such as a positive test, refusal to test, or other violation defined by DOT regulations. A SAP evaluation is required before an employee can begin the return-to-duty process in a DOT safety-sensitive role.
A DOT-SAP evaluation does not “clear” you by itself. The SAP provides a professional evaluation, makes education or treatment recommendations, and conducts a follow-up evaluation to determine compliance. The employer and the DOT return-to-duty testing process determine when an employee can return to safety-sensitive work.
During the initial evaluation, the SAP reviews the violation, gathers relevant history, and assesses current substance use and risk factors. The SAP then creates required recommendations for education and/or treatment and explains the steps needed to complete the DOT-SAP process.
The DOT-SAP process timeline varies based on the recommendations and how quickly education or treatment is completed. Some cases move faster with prompt scheduling and compliance, while others take longer depending on treatment needs and documentation requirements.
A return-to-duty test is a DOT drug and/or alcohol test required after the SAP process is completed and before returning to safety-sensitive duties. The employer (or designated agent) typically orders the return-to-duty test, and it must be a negative result to proceed.
Follow-up testing is a required plan of unannounced tests set by the SAP after the return-to-duty process begins. The SAP determines the frequency and duration of follow-up testing based on DOT requirements, and the employer is responsible for carrying out the testing plan.
